• NEW: Added a new feature on the DBA Utilities Tools menu to quickly set as "hidden" all ICD-10 codes that do not start with "F" (that is, hide all but the psych codes).
• FIX: The date selection in the Dx screen was not working correctly -- date selected on the calendar was not inserted in the field.

• FIX: (Optional Patient Import module). Several issues corrected. 1) If incoming data does not include a specified copay amount or percentage, the account will be set to 100% copayment.
• FIX: (Optional Encounter Import module). Corrected issue that caused a charge transaction to be created without any charngesplits and also create an adjustment that was not linked to a charge.

• FIX: Minibill and Superbill now print ICD-10 codes (if the service date is on or after your ICD-10 start date).
• FIX: Period Statements now print ICD-10 codes (if the service date is on or after your ICD-10 start date).
• FIX: Instant Claims now prints ICD-10 codes (if the service date is on or after your ICD-10 start date).
• FIX: (PRO Version only) Was using the wrong date to determine whether ICD-9 or ICD-10 codes should be printed. Also, we are now setting the ICD-10 indicator in block 66 correctly. You no longer have to set that value manually in the claim setup.
• CHANGE: (PRO version only)  The list windows for Professional and Institutional Claim Setups now display the ICD-10 rather than ICD-9 codes.
• FIX: Facesheet had been including Case Manager templates among the patients output. Also modified the report to use ICD-10 codes.
• FIX: (PRO version only) UB Institutional Claims were printing incorrect information in the CPT block 44 in some rare cases.
• FIX: (MAPSS/CBH module only) Posting service tickets to the daysheet was not populating ICD-10 codes.

• FIX: Changed the length of the Provider ID qualifier in block 17A from two to three characters for the Export CMS 1500 claim format.
• FIX: Corrected bug in the ICD-10 Prep Utility that was causing save errors if more than one user was working in the utility at the same time.

• FIX: enhanced the logic for the display of ICD9, ICD10, or both types of diagnoses, based on service date and settings of policies that are active on the service date and that are set to generate claims. Also fixed a warning that checked ICD9, but not ICD10, for missing Dx.
• NEW: Added ICD-10 Prep report that can be distributed to providers for completion.
• CHANGE: All paper claims now print all dates as MM DD YYYY except the dates in 24a, which will print as MMDDYYYY if the claim form selected specifies all 4 digit years.
• CHANGE: The CPT field in Services has been enlarged to handle up to 7 characters (up from 5).
• CHANGE: (Pro version) UB now has Supplier info in block 1 rather than Provider info.

• FIX: Provider Activity Reports (Primary and Rendering). The field size for subtotals of credit amounts was not long enough for larger values, resulting in trucation of the totals in the reports. 
• FIX: (Optional ERA posting module) Corrected some issues with automatic posting of payment from ERA (835) files. Posting can now succeed when a patient has one active and one inactive policy where both are members of the same carrier group. 
• FIX: The Unbilled Items report no longer includes unposted items in the current daysheet.
• CHANGE: The standard CMS 1500 claim form now uses 6 digit dates (MM/DD/YY) in blocks 12 and 31. Blocks 3 and 11a will continue to print 8 digit (MM/DD/YYYY) dates in accordance with NUCC documentation.
• NEW: Emdeon subscribers can now check eligibility from new utility available directly from Windows desktop, the Tools menu in OM, and from icon at the top of the patient policy list. See Electronic Claims manual for details.

• FIX: Display of OM Dx codes at the bottom of the DSM Profile screen now display ICD-10 codes. Also identified and fixed a bug that caused the wrong Dx's to display.
• FIX: Diagnosis section of the following reports was modified to support ICD-10 codes added in the DSM Profile: Intake Summary, Treatment Plan, Clinical Status Summary, and CM's Facesheet.

• FIX:  No longer prints empty pages, prints Rx history on a separate page, and suppresses Summary Notes in accordance with the report option specified by user.

• FIX: In the Pro version, it was possible for a user to delete a Treatment Episode, along with the related progress notes, treatment plan, therapy notes, and DSM profile. Deletion is now permitted only if there are no associated progress notes, treatment plan details, or therapy notes. 
• FIX: Treatment Plan Detail descriptions were being truncated in Treatment Plan and Progress Notes reports.
• FIX: Facesheet report included an invalid link to episodes.
• FIX: If, and only if, the Start/End Time feature is turned on in System Options for CM Progress notes, right-clicking in the progress note memo field jumps out of the memo to the start time field instead of displaying the edit context menu in the memo. 
• FIX: Error "cannot convert datetime" error appears if user adds a new patient, saves, then immediately re-opens the same patient to view or make changes.

• FIX: IMPORT PATIENTS module - If user did not provide a value for social security number, the importer would insert the value 000-00-0000 in the patient record. Changed to leave the value in patient blank.
• FIX:  IMPORT ENCOUTERS module - 1. If the service date is 10/1/2015 or greater, diagnoses will be treated as ICD-10 codes. 2. If the incoming service contains at least one Dx code, the services will be imported into the SOS Charge entry exactly as is. No additional codes will be copied into the charge from the Claim Setup.

• FIX: Corrected  a syntax issue that prevented a new trigger from being created when using the Rebuild utility in DBTools and when running the update.

12/13/2012

• FIX: (OM, CM) When choosing to add a file as an attachment in either OM or CM using the “Add and Delete” option, a confusing error appeared if the system did not have a program specified to handle that type of file. An example would be attaching a PDF file to a patient in SOS on a computer where there was no default program set up to use when opening PDF files. 

• NEW: (OM, CM) Added revision history (audit trail) for changes in patient User-Defined fields. It is now possible to query when changes in these fields were made, by whom, and the nature of the changes.

• NEW: Added revision history (audit trail) for most reports. It is now possible to query when these reports were run, by whom, and what selection criteria were input.

02/23/2012

• CHANGE: Removed confusing version information on the Help > About window in OM, CM, and Scheduler. Also added the build number to the Release information on the status bar at the bottom of the main screen in all three modules.

• FIX: Attachments in OM and CM could not be viewed if the path\filename contained a backslash followed by a lower-case "n". (\n was interpreted as a newline character). All filenames are now converted to uppercase to avoid this problem.

11/30/2011

• FIX: Automatic assignment of patient IDs was not working if one or more existing patients had non-numeric IDs.

• FIX: Database rebuild in Admin > DBA Utilities did not work in Windows 64.

• FIX: When attempting to set security permissions for for entire windows, there would be an error 33, preventing configuration.

• FIX: The admin module was changed so that you cannot change anything on the first two tabs of the user configuration of the user you used when logging in. In other words, it now works as it had in previous releases.

• NEW: Added menu item in DBTools allowing inspection of missing or extra database objects (triggers, tables, views, etc.) while users are in the system.

• FIX: In certain scenarios the database rebuild utility in Admin > DBA Utilities did not run.

• FIX: Autonumbering of ID's now works even if there is a mix of numeric and non-numeric ID's already in the system.

12/03/2010

• CHANGE: Both network and standalone versions of SOS now run the database with the DBSRV11 engine rather than using DBENG11 for standalone and DBSRV11 for network. This change should simplify installation, documentation, and upgrades. In addition it will eliminate certain errors that can appear from time to time in standalone implementations that are legitimately running several of the SOS modules and reports simultaneously. Starting with this release, the startup command is now in the form C:\SOS\SA\BIN32\DBSRV11.EXE  @C:\SOS\SERVER.PRM  C:\SOS\DATA\SOSDATA.DB. For standalone installations the SERVER.PRM file must contain the parameter -x none  rather than -x tcpip (which is the default if no -x is present).

• NEW: Added option in Admin > DBA Utilities to set query execution back to the way it was done in the previous version of SQL Anywhere. The "improved" technique used in SQL Anywhere 11 (standard in SOS 2010) has proven to be much slower for some customers. New installations of SOS 2010 will default this setting to the old technique. Users who want to experiment can do so using this option.

• FIX: The optional User Activity Logging feature was not recording or reporting the patient being accessed for many screens in the system.

07/28/2010

• CHANGE: The big change in release 2010 is the replacement of the older generation Sybase SQL Anywhere database engine (version 9) with the much-enhanced version 11. The new database engine should result in better performance and even more reliable service. 

• NEW: Along with rolling out the new Sybase version, we are also including 64 bit versions of the database engine for those with multi-gigabyte databases running on 64 bit servers. The primary benefit of using the 64 bit engine as opposed to the standard 32 bit engine is that it is much easier to configure a database cache larger than the 32 bit maximum of 1.8 GB. The steps required to switch to the 64 bit engine are available in a paper in the SOS web site document library.  (If the combined size of the DB files in your DATA folder are less than 2 GB, you might not see any benefit from switching to the 64 bit engine. Just be sure to adjust your startup parameters in SERVER.PRM to provide the appropriate amount of cache based on your database size and system resources. See this document library paper.

• CHANGE: Selecting Help > Online Resouces > Remote Support had previously immediately started the download of the customer-side connection software. An information and confirmation window has been added before the download step.

• NEW: Enhanced the scanner interface to allow setting options for document height/width, business card size, and duplex (two-sided) scanning when using the TWAIN interface. There is also an option to set whether to use TWAIN or the more limited WIA interface to communicate with your scanner.

• FIX: Admin > DBAUtilities had lost the option to terminate a user connection from the Watch Database tool. This function has been restored. In addition, the list of connections in this tool in both DBAUtilities and DBTools did not display the correct data in the correct columns, but now does.

• FIX: (Pro version) Changed font for paper UB04 form in boxes 76, 77, 78, 79 so that 10 digits will fit on the form in these fields. Changed  boxes 39a - 39d, 40a - 40d, 41a - 41d amount column so that dollar amounts print with two decimals and whole numbers print as integers, depending on your input for the associated Code field.
  
  
• FIX: Interest calculation was failing with an SQL error.
  
  
• FIX: Only four line item dx codes are permitted, so removed 8 of the 12 on the Charge Entry form.
  
  
• FIX: Removed prompt for Sex on Policy Form, Claim Form Top tab, box 9b. That field is not present on the new form.
  
  
• CHANGE: Removed extra vertical space on Charge Entry Form.
  
  
• FIX: ICD-10 rather than ICD-9 Dx's appeared on Charge Entry Form if patient had no insurance policies.
  
  
• FIX: Dx pointers that should have been numbers in electronic claims were set as letters instead.

• NEW: Added fields to support up to 12 ICD-10 dx's to Claim Setups, Charge Entry, and as appropriate, to claim and statement generators.
  
  
• NEW: Added carrier-level override date to Carrier/Plan form (so you can have an other-than-default change-over date for one or more specific carriers).
  
  
• NEW: Added item on the Tools menu in OM to import ICD-10's from ICD-10 Prep Utility to Claim Setups. There is also a button on the patient CSU windows to import just the ICD-10 codes for the current CSU (applies to professional (1500) and institutional (UB) claim setup screens).
  
  
• FIX: (Pro version)  Added <blank> to the droplist options for block 13 and 16 on the UB Claim Setup.
  
  
• NEW: Claims will now print ICD-9 codes for services rendered before the changeover date and ICD-10 codes after the changeover date. (changeover being the default in system options unless a different carrier-specific date has been set. If a batch contains services that cross the changeover date, separate ICD9 and ICD10 claims will automatically be generated for the appropriate services.
  
  
• NEW: Claims will contain the appropriate "ICD indicator" to show which type of Dx's are in use for each claim.
  
  
• NEW: Claims with ICD-10 Dx codes will use alpha pointers (A - L) on the service detail lines. Claims with ICD-9 codes will continue to use numeric pointers as before.
  
  
• NEW: (Pro version) ICD-10 code entries in UB claim blocks 67, 67A-Q, 69, 70A-C, 72A-C are now validated against the Dx code lists. If an invalid code (one that is not in the lookup list) is entered, the Dx lookup will appear.
  
  
• NEW: The charge entry window will show just ICD9 fields, ICD10 fields, or both, depending on the service date, the patient's active insurance policies, and the changeover date for each of the policy carriers.
  
  
• CHANGE: The old HCFA 1500 paper claim formats are now removed from the menus, but the files will still be shipped, and can be re-inserted in the claims menu, in the unlikely event that anyone should still need them. (These are the forms that were replaced in 2007 or so with the version we have been calling the "CMS 1500 form".)
  
  
• CHANGE: Updated the screen text in the professional (1500) Claim Setup screen to reflect the prompt on the new claim form.
  
  
• NEW: Added updated versions of the CMS paper format printouts to match the changes in the new "02-12" claim form. These are distinguished from the older versions by the presence of ("02-12") in the menu item descriptions.
  
  
• NEW: Created a new version of the "Export to CLAIMS.FIL,,," formatter to match the changes in the new "02-12" claim form.
  
  
• CHANGE:  (Pro version) The ID fields in blocks 76, 77 78, and 79 on the UB Claim Setup have been extended from 9 characters to 10 characters. In order to fit 10 characters in those fields on the paper form, these values now print in a slightly smaller font than the rest of the form.

------ Release 2013------

10/03/2013  (SOS 2013.02)

• NEW: Daysheet Print/Post now defaults to either "Mine" or "All" entries, based on whichever you selected last time you did a Print/Post.
• NEW: Added SOS ICD-10 Prep Utility to the Tools menu in OM. See User Guide in the Help menu of the utility. In SOS Office Manager Release 2013.02 or later, go to Tools >  ICD-10 Prep Utility. The utility will launch in a separate window.
• CHANGE: The Collections reports now include the selected provider code in the report page header.

08/16/2013

• FIX: If two users simultaneously open and exit a certain type of transfer transaction, the transaction no longer appears in the patient ledger.
  
• NEW: Added  Grand Totals section at the end of the Month End Summary report.
  
• FIX: The optional PT IMPORT utility was crashing with an MDI error if run in command line mode. The utility was also showing an Invalid Facility Number error even if no facility was included in the patient to be imported. If any errors are encountered, the error log will remain open to notify the user of the issues. If the patient is already present in SOS, the current data will be updated with the incoming data.
  
• FIX: The SortCode field was not being set in either the Refund Guide or the Returned Check Guide.
  
• NEW: Added field "Insurance Payment Group" on the Carrier form. By using that field to link two or more Carriers, the (optional) ERA Posting Module is now able to apply a single check across datasets and across insurance payers.
  
• FIX: "Show Payments" was including insurance payments made for other datasets.
  
• CHANGE: OM Lite can now access Lookups > Service Categories. The default printer setting for institutional claims has been disabled, as has the Provider Type option when adding Provider Fees. These two settings control features that are not available in the Lite version.
  
• NEW: (CBH version) Added alert for a service ticket that is entered with a date/time that overlaps a previously entered ticket for the same patient.
  
• FIX: Optional ENCOUNTER IMPORT utility was not importing the decimal portion of service units.

03/19/2013

• FIX: During insurance creation, some valid POS codes were being flagged as invalid.
  
  
• FIX: ExpressBill (optional product) now has a more responsive progress bar.
  
  
• FIX: Patient and Policy Attachments are now presented in date descending order (newest at the top). Previously they appeared in the order in which they were added to the list.
  
  
• FIX: When running the Ledger Report, the progress text and spinner was garbled.
  
  
• NEW: When entering a service macro in the Daysheet, the system will prompt you to apply pre-payments if there are any for the payors on the new charge entries.
  
  
• FIX: The Alternate Statements, including the By Charge, Open Item, and Express Bill (optional product) formats, now run much, much faster! In addition, the procedure that updates billed dates also runs much, much faster now.
  

01/31/2013

• CHANGE: The green "+" icon on the services list is now disabled when user is on the Macros tab. There is a separate icon used to add a macro, so the green "+" was just confusing.
  
  
• CHANGE: Macros are now converted to daysheet services in the exact order in which the services appear in the Macro screen. That is, the first service in the list (top) is the one that will be created first in the daysheet, followed by the one directly below that, and so on.
  
  
• FIX: Monthly finance charges could not be calculated. Stored procedure in the database has now been updated to reflect a change made in the 2013 release.
  
  
• FIX: Facesheet report now prints the alternate insurance address info, if a policy specifies an alternate address.
  
  
• FIX: Generating charges from Case Manager progress notes containing a macro service code was not working.
  
  
• FIX: (Optional ExpressBill module): Express Bill sometimes did not generate any statements for the "Alternate" statement format. 
  
  
• CHANGE: The Macro setup now contains an option to set the CSU default so it does not have to be done manually every time it is used.
  
  
• CHANGE: The "Tally MC auth by" setting on the Service has been changed. There is now an "Exclude" option that should be used with Add-On codes and other services that should not be included in the authorization usage counts. Please see: http://www.sosoft.com/userdocs/?p=319

12/13/2012

• NEW: Some users were encountering claim submission issues when filing claims for patients with punctuation in their names (eg: "O'Brien", "Smith-Jones"). If you encounter this issue, enter the patient name on the appropriate policy screen in the new fields supplied for this purpose. Anything you put in these fields will over-ride the entry/spelling of the name in the main patient screen, which does not allow certain punctuation.
  
  
• FIX: During daysheet posting, the list of undistributed checks was showing checks entered in other data sets.

• FIX: When there were several back-and-forth transfers on a charge, the More Splits feature did not work correctly.

• FIX: The lookup list when adding a Supervisor to a Provider showed providers in all datasets rather than just those in the current dataset. (Lookups > Providers, open Provider and click Additional tab. Field is "Supervising Provider".)
  
  
• NEW: Added button in toolbar in Services list (Lookups > Services) to create a duplicate of the currently selected service. When the new copy opens, you must change both the shortcut and the description before saving.
  
  
• NEW: Added variation of the Facesheet report. Identical to the current version, but the output contains only patients who are currently in the Active Patients list.
  
  
• NEW: Added button on toolbar of the ChargeSplit Entry Form so that you can Add, Change, or Delete claim adjustment reasons (CAR's) associated with the charge/payor. CAR's are required for electronic filing of secondary claims. They document what was paid or denied by the primary insurer.

------ Release 2010------

06/28/2012

• FIX: InstantClaim still had legacy TOS column header instead of current EMG in the detail section of the printout.

• FIX: [Optional ERA Posting Module] Was not displaying the check number in the detail list.

• FIX: [Optional ERA Posting Module, FL Medicare]  ERA's were not being downloaded to correct ERA_FLMC folder. Also fixed a problem with the OBTAIN ALL option for retrieving reports.

• NEW: Added item to Help menu to run AMMYY remote support tool now used most often by SOS support techs.

• NEW: The Undistributed Checks window that appears during posting of the daysheet now shows the ID of the user who entered the check, and the date it was added.

• FIX: [Optional ExpressBill Module] Certain values that should have been printed in the final statements were coming up blank.

• NEW: Added field on the Claim Adjustment Reasons form that allows entry and display of the ICN (essentially a claim ID number)  assigned by the primary carrier when it processes the claim. Some payers require this number to be included on the electronic secondary claim submission. 

• NEW: Added a column to the Patient Search function (binoculars icon on pt browse) to show the Data Set. Added a checkbox so the user can "Include all data sets" in the search. Leaving this box unchecked will only search in the current data set.

• NEW: Added a new stand-alone report called Provider Credits for Period. This report is the same as the provider credits summary found in the standard Provider Activity Report by Rendering Provider. It lists credits by type, with a breakdown by payor within each credit type.

• FIX: Although negative fees are not permitted in SOS, there were a couple of ways that could be used to change a fee to a negative amount.

03/25/2012

• CHANGE: Optional Express-Bill module now picks up the PO Box mailing address if present in the Supplier form.

• FIX: When scanning or adding document attachments, the option to store the document in the database and delete the original file was not doing the deletion. The document was imported successfully, but the original remained on disk as well.

03/11/2012

• FIX: When printing a claim for the secondary policy, the claim generator was pulling "other coverage" information from the current primary policy instead of the policy(s) that were actually primary at the time of the service.

• FIX: Paper claims did not select the correct Relation to Insured value if you selected one of the new 5010 relationship codes in the policy screen.

• FIX: The "Clear Insurance Billed Dates" feature was not showing (or clearing dates on) transfer charge splits.

• NEW: Added option to copy Managed Care Authorizations along with other policy and patient data.. To copy the auths, you must also copy the policies.

• NEW: To assist in entering CARs (claim adjustment reason codes) manually to do electronic submission of secondary insurance, the CARs form now provides a running total of the entered CARs and lets you know when you balance correctly.

02/23/2012

• NEW: Added a new paper claim format. If you have specified a separate mailing  address on the Supplier form (Lookups > Suppliers), this new format will print the mailing address instead of the main address in block 33 of the CMS 1500 claim form. This format can be used even if there is no separate mailing address, with results identical to "CMS 1500 claims".

• FIX: (Optional ExpressBill Module) Corrected "Error 30" resulting from failure by system to save the option for FTP and "Use NCFTP".

• NEW: (Optional ERA Module) Added link on page footer of report for the Code List on the web. If viewing the report on screen, the link is live -- clicking it will open the web page in your browser.

• FIX: Changed the claim generation setup option "Do not include other coverage info (box 9) on claims" to behave in the same fashion as the option on the Policy form "Determine Other Policy" when the latter is set "Force to None". Both now set the other insurance output to blank. Also corrected a bug that incorrectly output the "other insurance" specified by the primary policy's "Use specific policy" option. That scenario was marking both policies as primary in electronic filing.

• FIX: Account Period Summary (both By Provider and By Patient) - The report should suppress accounts with zeros in every column, but in some cases such accounts were appearing on the report because of computed non-zero values less than $0.01.

These reports present the same information as the Provider Activity Report by Rendering Provider, but in a different format that can be generated much faster than the original format.

• NEW: Added fields to Supplier form (Lookups > Suppliers)  for entry of  mailing address for use by electronic claim filers. If your mailing address is a PO Box, you must enter your PHYSICAL address in the main supplier address fields and the PO Box address in the separate Mailing Address fields to the right of the main address. If your mailing address is a regular street address and matches your physical address, check the Mailing address is same as physical address option, which will clear and disable the mailing address fields. If you file only on paper, you can continue as before, even if you have a PO box in the main address. 

• NOTE: Addresses in the Supplier and Facility screens should include a complete, 9 digit zip code. Failure to use 9 digit zips in these fields can result in claim rejections.

• CHANGE: Patient Note alarms are now checked as soon as you open the main OM window. There had been a delay of about 100 seconds, which has now been removed. Thereafter, notes are checked for alarms items once each minute.

• FIX: Under some circumstances, the UD Fields tab of the Patient Policies Form did not contain any fields. The system now checks for that situation and initializes the missing fields if they have not already been created.

• NEW: Added a warning if you try to save a new patient with no payor set to be responsible for charges. You can still do it, if necessary, but if there is no responsible payor, the system cannot automatically calculate charge splits. In that event, the splits have to be created manually when you enter a charge.

• NEW: Added two new variations of the Collections by Payor report. The new ones are the same as the existing Collections by Payor and Summary Collections by Payor reports except that you may enter a list of one or more Service Codes to be included in the results. All services other than the ones you enter are omitted from the report output.

• CHANGE: The Copy Previous list in the Charge entry form no longer includes any REFUND entries.

• FIX: When running Alternate Statements, a window appears asking if you want to update the billed dates for this statement run. Pressing ENTER was invoking the help system. The default button and action is now to execute the YES button when you use the ENTER key or SPACE BAR when that window appears.

• FIX: If you have more than one page of carrier exceptions for a service, scrolling up and down through that list could have resulted in duplicate rows appearing in the display (but not in the actual data). Scrolling no longer corrupts the list display.

• NEW: There is now an "Address Line Two" for the Insured in the Insurance Policy form. Data you enter here is ignored when printing paper forms, but is included in electronic claim submissions.

• CHANGE: ID Qualifier selection lists for providers, suppliers, referral sources, and facilities. PXC (Provider Taxonomy Code) has been added to the ID Qualifier selection list where appropriate.
